Elation Patient Passport is a secure patient portal for people who receive care at primary care practices that use Elation Health. Conveniently access your health information, manage care for your family members as a caregiver, and stay connected with your doctor's office.

With Patient Passport you can:

• Send and receive secure messages with your practice
• View upcoming and past appointments
• Cancel or book appointments online
• Read visit summaries after each appointment
• View your clinical profile
• Download, fax, or print your health records and immunization history
• Manage health records for family members under one login
• Update your demographics, preferred name, pronouns, and emergency contact
• Choose notification preferences by email or SMS

Available in English, Spanish, Chinese, Japanese, Korean, Tagalog, and Vietnamese.

Note: Patient Passport is available by invitation only. Your provider's office must send you an invitation to create an account. Available features depend on your practice's settings.
